The Grotto of the Sea Hags is a relatively unknown dungeon. It is located on a cliff far out on the Qalian coast in a chunk that is not only ten levels lower but remote enough from the major Qalian quest paths that few even go there in the first place . By the time most players have reached an appropriate level for it they have wandered far off and have little reason to go back. The quests for the Grotto have the same issue, as they are obtained at a low teens quest hub. Characters of appropriate level are unlikely to ever wander through and see that there are new quests available.
What this all boils down to is that if you wish to visit the Grotto, you're likely going to need to organize a group in advance. And of that group, probably none will have ever been there before and most will have never heard of the place. You'll port in to Temple of Dailuk chunk, ride a few minutes to Darkhorse Track to get the quests and then ride several minutes more to get to the dungeon. Once you get there you'll have the place to yourselves.
Is it worth the effort? Why yes. Yes it is.
The dungeon is a sea cave in a cliff overlooking the ocean. It has a shipwreck parked on top of it which implies a pretty serious storm or someone with a powerful motivation to move a shipwreck up to the top of a cliff. The interior is draped with seaweed, shells and strange growths. Old chests and broken rowboats seem to imply that the hags freely pick their choice of victims from passing ships. The ambiance is exactly what you expect it to be at the start and then twists into something much more sinister the deeper you progress.
The Grotto is not a large dungeon but it's a bit of a tricky one. Submerged tunnels and chambers, small passages between areas and several one way drops not only lend a bit of a mazelike feel to parts but also ensure that once you pass a certain point there is no return except by means of porting or cutting your way through to the other end of the place. Water breathing buffs are not essential but are strongly recommended.
Corridors are tight and the mobs are thick and very social. Pulls of a half-dozen or so 4 dot mobs are common. Crowd control is critical and a dedicated tank and healer are highly recommended. Each level deeper you travel increases the general difficulty of your opponents and each of the three levels of the dungeon has one quest aimed at it. Named mobs are everywhere and repop at a reasonable rate giving you ample chance to take them down as many times as you like.
The quests themselves are uninteresting and reward only coin and XP. The Grotto, however, tells its own story if you're paying attention and loot rewards are unnecessary as the itemization is extremely good. The abundance of named mobs makes for an abundance of rare drops filled out with plenty of uncommons. Several members of our group came out with almost entirely new sets of gear.
The Grotto is challenging, has great atmosphere and great loot. All of this plus its obscurity make this a great place for a low 20s group to visit somewhere that most players have never seen. Definitely recommended.

Lore
Only scraps are known of the Lore of the Grotto. It is occupied by a hideous bloated creature calling itself Peraxus the Sea Lord, a powerful Necromancer and also, apparently, a hedonist with some seriously wrong vices. He has surrounded himself with a horde of mummified zombie witches and they all hang out in areas such as the Chamber of Debauchery and the Pool of Hedonism. You really, really don't want to dwell on the implications here, especially after you've laid eyes on Peraxus who wallows in a pool known as the Cesspit of Peraxus. When not being entertained by his rotting harem, Peraxus apparently enjoys music as he has also set up a cabaret full of undead entertainers.
The surrounding areas of Qalia frankly don't spend much time dwelling on what goes on inside of the grotto. They're more concerned with the dangers to passing ships and the possibility of exploiting the grotto for knowledge and contraband.
